Effect of Ayurvedic Interventions on Uterine Artery Pulsatility Index In the Prevention of Preeclampsia-case Report

Abstract
Background: Pregnancy induces complex physiological changes, including substantial anatomical, hormonal, and haemodynamic adaptations essential for supporting foetal development and preserving maternal health. The uterine artery pulsatility index (PI), measured via Doppler ultrasonography, provides a non-invasive marker of utero-placental vascular resistance. While Ayurveda does not have a direct equivalent for this parameter, such clinical presentations may be correlated with Garbhajanya Vishamata (Toxaemia of Pregnancy) described in classical texts. Case Presentation: A 26-year-old primigravida (G1P0L0A0D0) at 6 weeks and 2 days gestation underwent an early anomaly scan, revealing a single live intrauterine gestation with normal Doppler parameters and no apparent congenital anomalies. Her antenatal course remained unremarkable until 22 weeks' gestation, when repeat ultrasonography identified high-resistance flow in the uterine artery with an elevated mean PI. Intervention: The patient received an integrative Ayurveda treatment protocol with formulations that are traditionally acknowledged for their vasodilatory, adaptogenic, anti-inflammatory, and hypotensive properties, supporting vascular relaxation, stress reduction, and cardiac function. Outcome:After one month of therapy, the patient exhibited a significant improvement in uterine artery PI, with serial assessments showing normalisation of Doppler indices. The intervention prevented progression to preeclampsia. She delivered a healthy female infant at term on 26 June 2025, with a birth weight of 2.2kg. Conclusion: This case illustrates the potential benefit of integrative Ayurveda management for raised uterine artery PI during pregnancy. While outcomes were favourable in this instance, further controlled studies are necessary to evaluate efficacy and safety on a broader scale.
