Ayurveda management of multi-drug resistant Perianal Abscess following surgical incision and drainage: A case report

Abstract
Background: Perianal abscess is an anorectal condition caused by infection of anal glands, often requiring surgical drainage and antibiotic therapy. Recurrent abscess and antibiotic resistance are growing challenges in modern clinical practice. Ayurveda offers wound management through Vrana Shodhana (cleansing) and Vrana Ropana (healing). Objective: To evaluate the effectiveness of Ayurveda management in a case of perianal abscess with antibiotic-resistant infection. Case Details: A 32-year-old male presented with throbbing perianal pain, hard stools, swelling below the scrotum for 5–7 days, and fever with chills. He was a known hypertensive on medications. Clinical examination and ultrasonography confirmed perianal abscess. Incision and drainage were performed under local anaesthesia, yielding 40–50 mL pus. Culture revealed multidrug-resistant organisms. The patient was then managed with Ayurveda treatment including Panchavalkala Kwath and Guduchi Kwath (30 mL twice daily), Laxmivilas Rasa (1 tablet twice daily), Gandhak Rasayan (2 tablets twice daily), and local wound care with Jatyadi Taila. Results: Significant improvement was observed within one week, with reduction in pain, discharge, and swelling. Complete wound healing was achieved within four weeks, with no recurrence noted. Conclusion: Ayurveda management demonstrated effective healing in perianal abscess with antibiotic resistance, supporting its role in wound care through cleansing and healing mechanisms.
