Ayurveda management of recurrent vaginal infection (Kaphaja Yoni Vyapada): A case report

Abstract
Background: Yoni Vyapada (Vaginal infection) is a broad term described in Ayurveda for various gynaecological disorders that arise due to the vitiation of Doshas (bodily humors), primarily affecting the female reproductive tract. Among these, Kaphaja Yoni Vyapada (recurrent vaginal infection) is characterised by symptoms such as Shweta Picchila Yoni Srava (thick, white, mucoid vaginal discharge), Kandu (itching), Alpa Vedana (mild pain), and features dominated by Guruta (heaviness) and Sheeta Guna (cold quality). In clinical practice, these manifestations are often comparable with vaginal infections such as vaginal candidiasis, trichomoniasis, and bacterial vaginosis. These conditions tend to recur and can significantly disturb a woman’s physical comfort, mental wellbeing, and overall quality of life. Objective: The study aimed to assess the effect of sitz bath with Triphala Kashaya and Yoni Pichu with Tila Taila in the management of Kaphaja Yoni Vyapada. In addition, it is sought to observe whether addressing associated psychological factors such as apprehension and anxiety through the administration of Brahmi Vati and Mansyadi Kashaya could contribute to holistic recovery and overall improvement in patient wellbeing. Case Details: A 31-year-old female patient presented with the chief complaint of intermittent white vaginal discharge for the past two years, associated with vulvar itching and foul smell. The patient also reported feelings of apprehension and psychological stress. Considering these symptoms, internal medications including Brahmi Vati and Mansyadi Kashaya were prescribed. Local therapeutic measures were also advised, which included Sitz bath with Triphala Kashaya and Yoni Pichu (intra vaginal tampon therapy) with Tila Taila, administered once daily for a period of seven days. Results: Following the intervention, the patient experienced noticeable improvement in her symptoms. There was a gradual reduction in the quantity of vaginal discharge along with relief from itching and foul smell. Conclusion: The use of Sitz bath with Triphala Kashaya and Yoni Pichu (intravaginal tampon therapy) with Tila Taila showed beneficial effects in the management of Kaphaja Yoni Vyapada. The addition of Medhya and Mano-prasadaka medicines also helped in addressing the psychological component of the condition, thereby supporting a more holistic recovery.
