Adjunct Effect of Katutumbi Taila Nasya (Nasal Instillation) in the management of Hypothyroidism: An exploratory clinical study

Abstract
Background: Hypothyroidism is a common endocrine disorder characterised by inadequate production of thyroid hormone. Hypothyroidism is a major metabolic disorder with Vata-Kapha Dosha Pradhana. Prevalence of hypothyroidism in female is more common than male. The thyroid gland is situated in Urdhwajatru (region above the clavicle), which is a Kaphasthana. Nasya may be a better option in the management of hypothyroidism. Katutumbi Taila has Ushna (hot potency), Teekshna (penetrating) and Kaphashamaka (Kapha pacifying) properties which are effective in hypothyroidism management. Objective: To evaluate the effect of Katutumbi Taila Nasya in different dosages on Thyroid Profile and BMI in patients with Hypothyroidism. Methods: Eight patients with serum TSH levels greater than 4.5μ IU/ml were enrolled in this study after fulfilling the inclusion criteria. All patients received Katutumbi Taila Nasya in two different dosages (4 or 6 drops in each nostril) according to the standard Nasya procedure for 7 days, followed by a gap of 23 days, and the procedure was repeated again for 7 days. Thyroid profile parameters (T3, T4 and TSH) and Body Mass Index (BMI) were assessed at baseline and after 60 days. Descriptive statistical analysis using mean and standard deviation was applied. Results: After completion of the treatment period, a reduction in mean serum TSH levels was observed in both groups. In Group 1 (4 drops), the mean TSH level decreased from 15.14 ± 8.25 μIU/ml to 9.64 ± 5.48 μIU/ml, while in Group 2 (6 drops), it decreased from 10.77 ± 6.89 μIU/ml to 6.81 ± 1.92 μIU/ml. T3 and T4 levels remained within the normal physiological range in most
patients. A mild reduction in BMI was also observed in both groups. Conclusion: Katutumbi Taila Nasya appears to be an effective and safe Panchakarma intervention in the management of hypothyroidism. No adverse effects were reported.
