Effect of Vidangadi Yoga (a Polyherbal formulation) on Metabolic and Body Composition Parameters in Pre-Diabetes: An Exploratory Pilot Clinical Study

Abstract
Background: Pre-diabetes represents an intermediate metabolic stage that increases the risk of developing type 2 diabetes and is frequently associated with obesity, dyslipidaemia, and insulin resistance. Although lifestyle modifications are considered the cornerstone of management, adherence to these measures is often inadequate in routine practice. From an Ayurveda perspective, this condition may be interpreted as part of a broader clinical spectrum which reflect an underlying metabolic imbalance, reflecting a shared underlying metabolic disturbance. Vidangadi Yoga is a classical polyherbal formulation in which the drugs collectively possess Lekhana Guna (Scraping properties); the formulation as a whole is Lekhana-Pradhana (predominantly scraping in action) and effective in the treatment of metabolic disturbance. Objective: The study was planned to explore the role of the trial drug on the above parameters in individuals with pre diabetes. Methods: Six pre-diabetic patients with HbA1c levels of 5.7–6.4% were taken and given Vidangadi Yoga with warm water in two different dosage regimens for 30 days. The effect of the intervention was assessed with Fasting blood sugar (FBS), Postprandial blood sugar (PP2BS), HbA1c, lipid parameters, and body composition mass index measured using the Omron HBF-T22 body composition monitor. Results: After the intervention, both groups showed reductions in blood glucose levels, with comparatively greater improvements seen in the higher dosage group. Lipid parameters showed mild improvements in both groups, with significant reductions in total cholesterol, triglycerides, LDL, and VLDL in the higher dosage group, while HDL remained unchanged. Body composition parameters showed positive changes in both groups, including slight reductions in body weight, BMI, and body fat %, along with small increases in muscle mass and resting metabolic rate. Overall, the higher dosage group showed better improvement in glycaemic, lipid, and body composition parameters. Conclusion: Vidangadi Yoga demonstrated beneficial effects on glycaemic control in pre-diabetic patients, with associated mild improvements in lipid profile and body composition parameters.
